Step 1 Initial Client and Agency Consultation
During this consultation, the client’s payor source, needs, preferences, and health status are discussed to gather essential information for creating a personalized care plan.
Step 2 Assessment & Care Plan Development
A tailored care plan is developed, outlining the services to be provided, frequency of care, and any specific goals for the client’s well-being
Step 3 Implementation of Services
Services are initiated as agreed upon. A caregiver is assigned, and the home care services begin. Regular check-ins and adjustments to the care plan may occur to ensure that the client’s needs continue to be met effectively.
